Hampshire County Council Public Health is working with local NHS organisations to redesign the way some health services for children, young people and their families are delivered across Hampshire. They would like to understand how existing services could be improved by bringing services together and making them easier to access.

Through the short survey, they want to hear from schools as well as parents/carers and young people across Hampshire, and gather their views on the following health services in Hampshire:
• Health Visiting Service
• School Nursing Service (not including special school nursing)
• NHS Community Therapy Services in Hampshire (i.e. speech and language, physiotherapy and occupational therapy)

They would be grateful if you could take time to complete the survey. The survey will be open until midnight on Tuesday 22 May 2018 and can be accessed via this link:

These surveys will be part of a number of ways in which we hope to collect feedback from the public and health practitioners.
The findings from the surveys and other accompanying engagement opportunities will help to inform decisions to be
taken later in the year about future service delivery for Public Health Nursing and NHS Community Therapies in Hampshire.
